Case Management
The goal of case management is to help clients maintain independence and control
over their lives and the decisions that affect them. This is achieved by linking
clients to much needed services. Coordination of services and continuity of
care are entrusted to a case manager who completes an in-depth assessment of
each
client's unique situation. This includes medical, social, psychological, and
survival needs as well as a review of medical, personal, financial, family,
and community resources. Those who are considered the client's families are
actively
involved in
the process and their needs for support and information are addressed as well.
Information and Referral
Provided by case managers and other staff. Information and referrals are available
to both clients and the general public utilizing our extensive resource files,
which include community services available to clients, up-to-date educational
materials on treatment/medical information, and information on community
programs and services available to our clients and their families. HIV
Prevention & Education
Strongest Link provides educational programs to assist people in learning ways
of protecting themselves from the spread of HIV. Our prevention programs focus
on providing updated information regarding HIV, modes of transmission, and
learning how to reduce high-risk behaviors. Professionally trained staff and
volunteers are available to provide these groups in a variety of settings.
In addition, people living with AIDS are available to educate the community
about the challenges of living with the disease.
